We are happy to announce that we have replaced traditional forms on clipboards with a new electronic check-in system that will streamline your check-in process. This is completed when you come to our office for your appointment. This new system is called Phreesia and takes only 10 to 15 minutes to complete.

Over the next couple of months, we will be asking that all of our patients check in via the Phreesia System. Once you have registered with the new system, the return visits only need to be verified and can be done within 3 to 5 minutes. The Phreesia System requires a four-digit pin number that you will use at every visit to register, so you will need to make it something you can remember. The new system also requires that you have your current insurance card. The system will verify that the card is current and active, as well as give us your correct co-pay.

Once you have set an appointment online or by phone, please download the forms listed below. Print the forms and bring them with you to your appointment. We have provided you with these forms to reduce your time in our reception area. If you prefer to complete the forms in our office please allow 15 minutes before your scheduled time to complete the new patient forms.
